License & Certification Survey

To gather information on licenses and certifications state employees hold pertaining to their current positions. Employees are critical in sustaining priority government services during any event that may threaten to disrupt those services.  With H1N1 Novel Influenza widespread in Minnesota, it is essential that emergency planners are able to quickly find employees with the knowledge and licensure necessary to assist in maintaining these critical services.  You, as a state employee, are key to the State of Minnesota maintaining those services to the citizens of Minnesota.

No.  Employees are strongly encouraged to complete this survey so emergency planners have the data they need to develop strategies to continue priority government services.

Employees will not be disciplined for not responding to the survey.  However, not receiving information from employees may affect emergency planners’ ability to identify staffing shortages and create realistic plans for providing priority services.

The license and certification information is defined as public data by the licensing boards that issues your license and/or certification. The information collected by the survey will be used for emergency and continuity planning purposes and viewed by those involved with that activity.

In times of high absenteeism state agencies must quickly find and deploy personnel with the appropriate knowledge and licensure to ensure that priority services are provided. The license and certification information will be used to find the employees needed to keep priority services running.
